Community Care Specialist

About the Role:
We are seeking a skilled Registered Practical Nurse to join our team as a Community Care Specialist. In this role, you will provide high-quality patient care in various settings, working with patients of all ages and complexity of care needs.
Key Responsibilities:
- Deliver scheduled visits to patients in their homes, providing personalized care and support.
- Use assessment tools and techniques to identify individualized patient needs and develop effective care plans.
- Execute and monitor care plans, ensuring patients receive the best possible care.
- Build strong relationships with patients and their families, promoting trust and rapport.
- Maintain accurate and up-to-date patient records, documenting all patient encounters and care plans.
Requirements:
- Registration with the CNO as a Registered Practical Nurse in good standing.
- Valid driver's license and access to a vehicle.
- Experience in delivering community care is an asset.
- Reliability, flexibility, and a passion for making a difference in your community.
