About the Role:

We are seeking a skilled Registered Practical Nurse to join our team as a Community Care Specialist. In this role, you will provide high-quality patient care in various settings, working with patients of all ages and complexity of care needs.

Key Responsibilities:

  • Deliver scheduled visits to patients in their homes, providing personalized care and support.
  • Use assessment tools and techniques to identify individualized patient needs and develop effective care plans.
  • Execute and monitor care plans, ensuring patients receive the best possible care.
  • Build strong relationships with patients and their families, promoting trust and rapport.
  • Maintain accurate and up-to-date patient records, documenting all patient encounters and care plans.

Requirements:

  • Registration with the CNO as a Registered Practical Nurse in good standing.
  • Valid driver's license and access to a vehicle.
  • Experience in delivering community care is an asset.
  • Reliability, flexibility, and a passion for making a difference in your community.
