Job Summary

We are seeking a skilled Social Worker to join our Primary Care team in Oliver. This is a permanent full-time opportunity to work with diverse populations and evolve the practice of social work in a dynamic environment.

Key Responsibilities
  • Provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families.
  • Work collaboratively with primary care providers and the interdisciplinary team to provide direct psychosocial care, counseling, and group work.
  • Facilitate client self-management and provide linkage to relevant specialized complex care resources.
  • Maintain client records and provide operational, policy, and ethical practice recommendations.
Requirements

The successful candidate will have a Bachelor's degree in Social Work and experience working with diverse populations. Strong communication and interpersonal skills are essential for this role.
