Forensic Community and Liaison Social Worker

Job Summary
We are seeking a highly skilled Forensic Community and Liaison Social Worker to join our team at BC Mental Health and Substance Use Services. As a key member of our team, you will work closely with clients, families, and healthcare providers to provide comprehensive social work services in a forensic mental health setting.
Key Responsibilities
- Conduct initial intake and ongoing biopsychosocial assessments to identify client needs and develop integrated treatment plans.
- Provide community case coordination and advocacy services to support clients in accessing community resources and services.
- Maintain accurate and concise documentation of client information and provide written reports as required.
- Collaborate with healthcare providers and community agencies to ensure comprehensive care and support for clients.
- Facilitate access to psychiatric and social services through referrals and site visits.
- Consult with the clinical treatment team to determine client needs and develop strategies for client reintegration and prosocial development.
Requirements
- Bachelor's degree in Social Work from an accredited School of Social Work.
- Two years recent, related experience working in a mental health setting.
- Full registration with the British Columbia College of Social Workers (BCCSW) is required.
- Valid BC Drivers License.
- Current Cardiopulmonary Resuscitation (CPR) Certificate.
