Job Summary

The Registered Practical Nurse is a vital member of our healthcare team, responsible for providing high-quality, client-centered care to individuals and families in their homes and community. As a Registered Practical Nurse, you will work collaboratively with our multidisciplinary team to deliver evidence-based care, promote health and independence, and support clients in achieving their goals.

Key Responsibilities
  • Provide nursing care to clients with predictable needs, utilizing established plans of care and consulting with RNs as needed.
  • Manage and facilitate care through established nursing care plans, evaluating client responses to interventions and making adjustments as necessary.
  • Provide cost-effective, organized, and outcome-directed care on an ongoing basis, prioritizing client needs and preferences.
  • Conduct comprehensive assessments of clients and their environments, utilizing established procedures and documenting findings clearly and concisely.
  • Collaborate with clients, families, and healthcare teams to develop and implement care plans, promoting client-centered care and independence.
  • Document all relevant information accurately and appropriately, adhering to VON standards and guidelines.
Requirements
  • Current licensure as a Registered or Licensed Practical Nurse in the province of practice.
  • Experience in community health and clinical practice is an asset.
  • Strong communication and interpersonal skills, with the ability to work collaboratively in an interdisciplinary team.
  • Ability to problem-solve and adapt to changing client needs and priorities.
Working Conditions

This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, and other movements that emphasize good body mechanics. Individuals in this role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
