Key Responsibilities
* Provide primary health care services to patients in the community
* Work collaboratively with other healthcare professionals to ensure seamless care delivery
* Conduct assessments and develop care plans tailored to individual patient needs
* Communicate effectively with patients, families, and caregivers to ensure informed decision-making
* Participate in quality improvement initiatives to enhance patient outcomes and experiences
Requirements
To be successful in this role, you will require:
* A Bachelor of Science in Nursing (BScN) degree
* Current registration as a Registered Nurse Practitioner (RN(NP))
* Certification in a recognized Primary Care NP equivalent program
* Basic Cardiac Life Support Certificate (BCLS)
* One year of experience in a Primary Health Care setting preferred
* Strong knowledge of population health approaches to health promotion and disease prevention
* Demonstrated ability to work effectively in a fast-paced environment and prioritize tasks accordingly
* Excellent communication and interpersonal skills
