Nurse Case Manager

We are seeking a highly skilled and compassionate Nurse Case Manager to join our team at CB Canada. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support.
Key Responsibilities- Provide Medical Support: Offer education and guidance to patients and healthcare providers on specific diseases and therapies, as well as disease management.
- Telephone Support: Provide maintenance calls and adherence support to patients, as needed.
- Database Management: Manage and audit patient files and information captured.
- Case Management: Administer and adhere to case-management protocols, including clinical monitoring, data collection, patient follow-up, compliance, therapy disruption/discontinuation monitoring, adverse event reporting, and liaison with primary healthcare providers.
- Training and Development: Participate in extensive training sessions for specific therapies provided by the pharmaceutical team, as required.
- Reimbursement and Funding: Work with the reimbursement specialist and physician to secure funding for patients' therapies and assist with alternative funding searches when needed.
- Communication and Liaison: Establish and maintain partnerships with pharmaceutical teams, healthcare providers, pharmacies, infusion clinics, and other agencies to coordinate infusions, drug delivery, and resolve issues as they arise.
- Team Collaboration: Collaborate with patient associations and advocacy groups, as required, and participate in the training process of new nurse case managers.
- Education and Experience: A Bachelor's degree or registered nurse (RN) diploma, advanced degree, or specific practice management experience preferred, with five (5) years of Nursing experience.
- Relevant Skills and Knowledge: Strong patient focus, ability to multi-task, work under pressure, and provide superior customer service. Proficient in internet research, computer skills, and strong verbal and written communication skills.
