Job Summary

We are seeking a highly skilled and experienced Senior Practice Consultant to join our team at Alberta Health Services. As a key member of our Seniors Health program, you will provide advanced subject matter expertise in Continuing Care (clinical and program development) to support clinical operations teams.

About the Role

The successful candidate will work closely with our executive director and other stakeholders to provide specialized consultation services, coordinate and implement multiple SH programs and services, and manage complex clinical problems. This role also involves leading and managing a multidisciplinary team of leaders, including administration staff and Leads for Client Directed Care programs.

Responsibilities

  • Provide advanced subject matter expertise in Continuing Care (clinical and program development)
  • Support clinical operations teams with complex clinical problems
  • Manage and lead a complex multidisciplinary team
  • Coordinate and implement multiple SH programs and services
  • Establish and maintain effective working relationships with stakeholders

Requirements

  • Active or eligible for registration and practice permit with a regulated body
  • A minimum of a bachelor's degree in a health-related field and/or a Senior Practice Consultantship Stream or equivalent experience in Seniors Health related work
  • Minimum of 7 to 10 years of health care experience
  • Minimum of 5 years work in Seniors Health (home and community care, CCH Type A, CCH Type B, palliative care)
  • Connect Care, and RAI-HC trained
  • Case Management experience
  • Competent and comfortable with MS office suite and virtual meeting venues (Teams, Zoom etc.)
  • Evidence of education development or continuous learning

Preferred Qualifications

  • Advanced education (e.g., master's degree) or certification in an appropriate clinical specialty (e.g., Canadian Nurses Association certification in Community Health, Gerontology, NSWOC and/or Hospice Palliative Care) or similar
  • PIECES and/or Supportive Pathways Training

What We Offer
