Care Coordinator

The Nova Scotia Health Authority is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in assessing, planning, and authorizing services for clients referred to Continuing Care. Your expertise will help individuals and families navigate complex health situations and access the resources they need to thrive in their communities.
Key responsibilities include:
- Conducting comprehensive assessments to identify client needs and goals
- Developing and implementing care plans that address client needs and promote independence
- Authorizing services and making referrals to community resources and healthcare teams
- Collaborating with interprofessional teams to facilitate hospital discharge and community reintegration
- Maintaining ongoing communication with clients, families, and healthcare providers to ensure seamless care coordination

About YouWe are seeking a candidate with:
- A Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
- Current registration with the applicable Nova Scotia Professional Regulatory body
- Minimum three years' experience in a community or long-term care setting
- Demonstrated knowledge and experience in dedicated case management or discharge planning
- Excellent assessment, interviewing, and negotiation skills
- Competent computer skills and ability to conduct assessments using a laptop
This is a permanent, full-time position with 70 hours bi-weekly. Shifts may include days, evenings, weekends, and holidays.
Compensation and IncentivesThe successful candidate will receive a competitive hourly rate of $41.06 - $46.75, as well as access to our comprehensive benefits package, including health, dental, travel, long-term disability, and life insurance coverage, and a defined benefit pension plan.
