Housing First Case Manager Job Description

The Salvation Army is seeking a highly skilled and compassionate Housing First Case Manager to join our team. As a key member of our housing support services, you will play a vital role in helping individuals and families achieve stable and secure housing.

Key Responsibilities:
  • Provide comprehensive case management services to clients, including assessment, goal setting, and service planning.
  • Develop and implement individualized housing stability plans, focusing on meeting basic needs, safety, and housing tenure.
  • Conduct regular outreach and engagement with clients, including those living in poverty, dealing with housing and homelessness issues, and/or mental health sectors.
  • Collaborate with community resources, agencies, and stakeholders to ensure seamless service delivery and support.
  • Maintain accurate and up-to-date client records, including case files, documentation, and data collection.
  • Participate in ongoing training and professional development to enhance skills and knowledge in housing first strategies, crisis intervention, and trauma-informed care.
Requirements:
  • Completion of a two-year Community College Diploma in a Social Services or Addictions-related field.
  • Minimum of two to three years of experience working with vulnerable populations, including those experiencing homelessness, poverty, and mental health issues.
  • Knowledge of homelessness and affordable housing issues, as well as Housing First strategies to end homelessness.
  • Strong communication, interpersonal, and organizational skills, with the ability to work independently and as part of a team.
  • Ability to work in a fast-paced environment, prioritize tasks, and manage multiple cases simultaneously.
Working Conditions:
  • This is a full-time position based on 37.5 hours per week.
  • The Supervisor will set the hours of work and work schedule.
  • Working environment is typically in the office or out in the community in generally agreeable conditions.
