Housing First Case Manager Job Description

The Salvation Army is seeking a highly skilled and compassionate Housing First Case Manager to join our team. As a key member of our housing support services, you will play a vital role in helping individuals and families achieve stable and secure housing.

Key Responsibilities:
  • Provide comprehensive case management services to clients, including assessment, goal setting, and service planning.
  • Develop and implement individualized housing stability plans, focusing on meeting basic needs, safety, and housing tenure.
  • Conduct regular outreach and engagement activities to identify and support individuals and families at risk of homelessness.
  • Collaborate with community partners and stakeholders to ensure seamless service delivery and effective resource allocation.
  • Maintain accurate and up-to-date client records, including case notes, service plans, and progress reports.
  • Participate in regular team meetings and training sessions to enhance skills and knowledge.
Requirements:
  • Completion of a two-year Community College Diploma in a Social Services or Addictions-related field.
  • Minimum of two to three years of experience working with individuals and families experiencing homelessness or poverty.
  • Knowledge of Housing First strategies, residential tenancy act, and community resources.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team.
Working Conditions:
  • Full-time position based on 37.5 hours per week.
  • Typically works in an office or community setting, with occasional travel required.
  • Ability to lift/move up to 25 pounds.
