About the Role

Circle of Care is seeking a skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in providing high-quality support services to Holocaust Survivor clients, ensuring their needs are met and their well-being is prioritized.

Key Responsibilities

  • Respond to client inquiries and provide guidance throughout the application process
  • Conduct in-home and in-office visits to support client applications and assessments
  • Assist clients in collecting necessary information and documents for program applications
  • Develop and implement care plans in collaboration with clients and caregivers
  • Provide counseling and informal support as needed
  • Coordinate with healthcare providers and community resources to ensure clients receive optimal care
  • Maintain accurate and up-to-date client records

Requirements

  • Master's degree in social work, nursing, or a related field
  • Minimum 1-3 years of clinical experience, preferably in community care or case management
  • Experience working with geriatric populations, Holocaust Survivors, and/or survivors of trauma
  • Strong knowledge of community health providers and home care options
  • Excellent organizational, communication, and interpersonal skills
  • Ability to work independently and collaboratively as part of a dynamic team
