Job Summary:

The Personal Support Worker provides personalized care and support services to clients and families within the framework of VON Canada's policies and procedures. This role involves collaborating with healthcare teams to meet the needs of clients and promote their independence and well-being.

Key Responsibilities:

  • Assess and evaluate client needs to develop and implement personalized care plans.
  • Provide personal care and support services, including light housekeeping, meal preparation, and personal hygiene assistance.
  • Communicate effectively with clients, families, and healthcare teams to ensure seamless care delivery.
  • Monitor and maintain client safety and well-being, reporting any concerns to supervisors.
  • Participate in multidisciplinary teams to coordinate care and services.

Requirements:

  • High school diploma or equivalent.
  • CCA Certification or equivalent required.
  • Experience working with elderly or ill individuals or children.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
