Job Summary

The Home Care Specialist provides personalized support and homemaking services to clients and families within the framework of VON policies, procedures, and standards. This role involves collaborating with healthcare professionals to meet the needs of clients and promote their independence and well-being.

Key Responsibilities
  • Conduct thorough assessments of clients' homes, personal situations, and care plans to identify areas for improvement and ensure their needs are met.
  • Develop and implement personalized care plans in collaboration with the Personal Support Program Manager.
  • Provide personal care services, including light housekeeping, meal preparation, and assistance with daily living activities.
  • Monitor and maintain household safety, and report any concerns to the supervisor or Personal Support Program Manager.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in multidisciplinary teams to provide comprehensive care and support.
  • Perform administrative tasks, such as completing daily call checks and other forms as required.
Requirements
  • High school diploma or equivalent.
  • Secondary school education is preferred.
  • CCA Certification is required in Nova Scotia.
  • Must have a certificate in Home Support Worker (Level I, II, III), Personal Support Worker program, or Health Care Aide.
  • Must have a current Standard First Aid Certificate or CPR level C Certificate, if applicable to the work being performed.
  • Some household management experience and experience working with elderly or ill individuals or children.
Skills and Abilities
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a multidisciplinary team or independently as required.
  • Good assessment and problem-solving skills.
  • Awareness of community resources and ability to connect clients with relevant services.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
Work Environment
