Job Summary

Vancouver Coastal Health is seeking a skilled Social Worker to join their team in Vancouver, BC. As a Social Worker with VCH, you will provide advanced clinical and consultative social work services to clients and families, focusing on how the client's relationships are affected by their environment.

Key Responsibilities
  • Conduct social and psychosocial assessments, providing individual, family, and group counseling services
  • Maintain client records, attend interdisciplinary clinical/case conferences, and identify applicable community resources for clients
  • Facilitate care throughout the continuum, ensuring seamless transitions and optimal outcomes
Requirements
  • Current full registration with the British Columbia College of Social Workers (BCCSW)
  • Master of Social Work from an accredited School of Social Work and one (1) year post-Bachelor of Social Work experience in a healthcare setting
