Your Role: As a Speech Language Pathologist, you'll play a vital role in helping clients achieve their full potential by developing and implementing personalized speech therapy plans. You'll work closely with healthcare professionals to ensure seamless care coordination and effective goal achievement.

Responsibilities:

  • Conduct thorough assessments to identify clients' communication needs and develop tailored treatment plans.

  • Collaborate with interdisciplinary teams to ensure comprehensive care coordination and effective goal achievement.

  • Participate in the SE Transitions program, leading care planning and facilitating team collaboration to reach clients' care goals.

Requirements:

  • A graduate degree in Speech Language Pathology from an accredited program and membership in good standing with the College of Audiologists and Speech-Language Pathologists of Ontario.

  • Recent experience in managing patients in acute/rehab or community-based rehabilitation programs is preferred.

  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

  • Ability to adapt to new computer applications and technologies.

  • A positive work and attendance record is essential.
