Job Description

As a Registered Practical Nurse with SE Health, you'll have the opportunity to work in a dynamic and supportive environment, collaborating with a multidisciplinary team to deliver high-quality care to our patients. Your primary responsibilities will include:

  • Completing comprehensive client assessments and developing personalized care plans
  • Implementing and evaluating care based on best evidence and practices, as well as client-specific needs
  • Supporting clients through care transitions by connecting them with community resources and services
Requirements

To be successful in this role, you'll need to possess:

  • A current registration with the College of Nurses of Ontario
  • Canadian Nurses Association (CNA) specialty certification, if applicable
  • Enthusiasm for community care and passion for evidence-informed practice and excellence in care
  • A valid driver's license and vehicle, as travel is required
  • Updated immunization records, clear criminal background check, and Vulnerable sector check, as required

Return of Service Program

To be eligible for the Return of Service program, you must meet the following criteria:

  • Not have been employed as a nurse in Ontario in the six months prior to being hired
  • Hold a Certificate of Registration in good standing from the College of Nurses of Ontario
  • Begin employment no later than March 31, 2025
  • Commit to 24-months of full-time employment at a guaranteed schedule offered by the employer
  • Not simultaneously receive funds from the Ministry of Health for the Tuition Support Program for Nurses
