Care Coordinator for Older Adults

We are seeking a compassionate and skilled Care Coordinator to join our team at Verve Senior Living. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality care for our residents.

Key Responsibilities
As a Care Coordinator, your primary responsibilities will include:
- Assessing the needs of individual residents and developing personalized care plans to promote their independence and well-being.
- Collaborating with healthcare professionals, families, and other stakeholders to ensure seamless care coordination.
- Supervising and guiding caregivers to provide high-quality care and support.
- Distributing medications and administering treatments as per approved standards of care.
- Responding to emergency situations and providing immediate assistance as needed.
- Maintaining accurate records of nursing observations and procedures performed.
- Participating in continuous quality improvement initiatives to enhance resident care and satisfaction.
- Providing ongoing support to co-workers and advocating for residents' rights and dignity.
Requirements
To be successful in this role, you must possess:
- A minimum of grade 12 education and a recognized nursing license (RPN) in good standing with the College of Nurses of Ontario (CNO).
- Certifications in Emergency First Aid, CPR, and AED.
- Previous experience working as an RPN in a healthcare setting, preferably in a senior living environment.
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
- Ability to work independently and as part of a team, with a strong focus on customer service and resident-centered care.
- Proficient computer skills, including Microsoft Office applications.
What We Offer
In addition to a competitive salary range of $65,000 - $85,000 per year, depending on experience, we offer a comprehensive benefits package, opportunities for professional growth and development, and a dynamic work environment that values teamwork and collaboration.
