Job Summary
We are seeking a highly skilled and compassionate Pharmacist to join our team as a Patient Care Specialist. As a key member of our pharmacy team, you will be responsible for providing exceptional patient care, counseling, and education to patients and caregivers.
Key Responsibilities
- Provide patient-centered care, including medication reviews, health risk assessments, and disease state management.
- Counsel and educate patients and caregivers on new therapies, medications, and health-related topics.
- Initiate care plans, follow up, and complete proper documentation.
- Participate in professional programs and collaborate with colleagues to provide excellence in health and wellness services.
- Ensure prescription safety and prioritize technical functions to Pharmacy Registered Technicians and Assistants as required.
- Thoroughly understand all relevant corporate pharmacy service offerings and provide each customer with the program that meets their healthcare needs.
- Maintain current knowledge of pharmacy practice, regulatory requirements, and industry trends.
Requirements
- Bachelor of Science in Pharmacy from a recognized institution.
- Licensed Pharmacist, in good standing, in the province of responsibility.
- Injection and First Aid Certification.
- Kroll software experience is preferred.
