Job Title: Mental Health and Addiction Case Coordinator

We are seeking a highly skilled and compassionate Mental Health and Addiction Case Coordinator to join our team at Providence Health Care. As a key member of our interdisciplinary team, you will provide flexible and inclusionary treatment and intensive case management services to individuals, primarily youth and young adults, with mental health and addiction issues.

Key Responsibilities:
  • Provide outreach services to youth and young adults with chronic psychiatric issues, including mental health and addiction assessments, treatment planning, and crisis intervention.
  • Establish and maintain therapeutic relationships with clients and their informal support networks, promoting empowerment and self-care.
  • Collaborate with interprofessional team members to ensure optimal plan of care is implemented, and provide education and training to clients and their families.
  • Participate in case conferences and meetings to identify mental health and addiction issues, coordinate services, and ensure continuity of care.
  • Act as a client advocate to protect and promote clients' rights to autonomy, respect, privacy, dignity, and access to information.
Requirements:
  • Registered Nurse with current practicing registration with the British Columbia College of Nurses and Midwives or a Registered Psychiatric Nurse with current practicing registration with the British Columbia College of Nurses and Midwives.
  • Two years' recent, related experience working in youth and young adults with mental health and addiction in a community health setting.
  • Demonstrated knowledge of nursing theory and practice related to mental illness and addiction.
  • Ability to provide treatment planning, crisis intervention, and case coordination.
  • Ability to manage complex clinical and psychosocial crises in the identified population.
