About the Job

Frazer Health is currently seeking a highly skilled and compassionate Mental Health Care Coordinator to join our team. As a key member of our mental health care team, you will play a vital role in coordinating and delivering high-quality care to individuals experiencing acute psychiatric crises or situational emergencies.

Key Responsibilities:

  1. Develop and implement comprehensive care plans that address the unique needs of each client.
  2. Collaborate with interdisciplinary teams to ensure seamless coordination of care services.
  3. Provide education and support to clients and families on mental health conditions, treatment options, and community resources.
  4. Monitor and evaluate the effectiveness of care services and make recommendations for improvement.

Requirements:

  • Graduation from a recognized nursing program with current registration as a RN/RPN.
  • Minimum one year of experience working in mental health and substance use settings.
  • Strong communication and interpersonal skills with ability to work effectively with diverse populations.
