As a Registered Nurse/Registered Psychiatric Nurse, Aboriginal with Fraser Health, you will play a vital role in providing clinical mental health nursing services to Aboriginal clients in community programs. Working collaboratively with the interprofessional care team, you will assess clients' mental status, formulate plans of care, and implement planned care to achieve positive outcomes.
Key Responsibilities- In consultation with local community Aboriginal health/mental services/agencies, work to enhance existing services and develop new initiatives that cater to the unique needs of Aboriginal clients.
- Function as a primary nursing therapist and case manager, providing diagnostic client assessments and short and long-term individual, group, and family therapy using various interviewing, counseling, and assessment methods.
- Liaise between acute care hospitals, community mental health and addictions services, and Aboriginal community agencies to facilitate seamless transitions and optimize patient care.
- Collaborate with healthcare professionals, counsellors, traditional healers, and Elders to provide culturally sensitive education and support to patients and their families.
- Develop consultative relationships with service providers to ensure that cases are handled in a culturally sensitive manner and that clients receive comprehensive support.
- Facilitate referrals to appropriate services, including mental health programs, shelters, residential/housing services, and medical clinics, ensuring that clients have access to necessary resources.
- Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Provide culturally sensitive educational materials to local Mental Health Centre staff and agencies working with Aboriginal clients with mental illness.
