Clinical Social Worker Home Health New Westminster

The Fraser Health Authority is seeking a skilled Clinical Social Worker to join our Home Health team in New Westminster. As a member of our interdisciplinary team, you will provide assessment and treatment services to clients, liaise with community agencies, and participate in educational services, planning, and program development.
Responsibilities- Provide therapeutic counseling services to clients and families experiencing complicated circumstances related to various medical diagnoses.
- Conduct social work and psychosocial assessments of clients and families, interpreting the information using various modalities in accordance with professional standards and clinical practices.
- Follow up and provide ongoing monitoring of complex psychosocial situations, evaluating clients' responses to interventions.
- As a member of the interdisciplinary team, provide information regarding clients' psychosocial situations and participate in developing a comprehensive plan of care with/for the client, collaborating and consulting with other health service area professionals on the care plan.
- Act as a resource for clients and their families by providing information regarding community resources and access to these resources, facilitating referrals where appropriate, and facilitating the development of HSA resources by identifying services needed to support the health of families.
- Participate in quality improvement activities by providing input into the development, implementation, and evaluation of program goals, objectives, policies, and procedures.
- Promote client autonomy by ensuring their participation in the planning, decision-making, and development of problem-solving abilities.
- Assist in the development and provision of relevant educational programs for home health care staff, clients, their families, and community agencies on psychosocial issues related to health, illness, and disability.
- Prepare statistics, in accordance with the workload measurement system, for submission to designated personnel, and maintain written clinical records as required.
- Perform other related duties as assigned.
Education and Experience:
A Master's Degree in Social Work from an approved school of Social Work. Two (2) years' recent related experience in a complex health care environment such as a community or home health setting, or an equivalent combination of social work education, training, and experience.
Current full registration with the BC College of Social Workers. Valid BC Driver's License and access to personal vehicle for business-related purposes.

- Demonstrated ability to conduct social and psychosocial assessments.
- Ability to provide leadership to the interdisciplinary team in care planning in complex client situations.
- Ability to work effectively with others, including working effectively in a multicultural environment.
- Ability to work collaboratively as part of an interdisciplinary team and understanding the scope of practice of other health care disciplines.
- Ability to make effective referrals, reflecting in-depth knowledge of community resources and strong collaborative skills in working with other community agencies.
- Demonstrated ability to communicate effectively both verbally and in writing with clients, families, other members of the health care team, and community agencies.
- Demonstrated ability to effectively manage conflict.
- Ability to facilitate group meetings, processes, and decision-making.
- Ability to operate related equipment, including related computer software.
- Physical ability to perform the duties of the position.
