Stroke Care Services Coordinator

This role is a key part of our community stroke care service, working closely with healthcare professionals to provide coordinated, efficient, and timely access to care services for patients and their families.
Responsibilities- Determine program eligibility and assess the need for care at home, alternative living environment, or in a personal care home.
- Initiate referrals and service plans for the delivery of professional and other health services based on operational directives and protocols guidelines.
- Continuously be responsible for client assessment, counseling, education, developing/planning, care needs, coordinating, and ensuring delivery of recommended community-based client services throughout the client's participation in home-based rehabilitation.
- Minimum two years' experience in acute or rehab hospital and/or community-based service delivery required.
- Prior experience in neurology, stroke care, and/or rehabilitation strongly recommended.
- Experience as a Case Coordinator in Community Health Services is an asset.
- Recent and relevant continuing professional education in a related clinical area preferred.
- Bachelor's degree in a relevant field (BSW, OT, PT, BSc PN, RPN, BN, RN, SLP, RD) required.
- Entry-to-practice academic credentials in a relevant accredited Canadian University program or approved equivalent if internationally educated as determined by the incumbent's respective Manitoba College.
- Competitive salary range: $36.71 - $50.64 per hour.
