Job Summary

We are seeking a highly skilled and compassionate Clinical Social Worker to join our team. As a key member of our interdisciplinary team, you will provide professional social work services and interventions to clients with severe and persistent mental illness.

Key Responsibilities
  • Provide individual, family, and group therapy and interventions to clients, including psychosocial assessment, risk assessment and management, crisis intervention, and discharge planning.
  • Develop and implement creative treatment, recovery, and therapy plans that are responsive to clients' needs and aligned with best practices.
  • Engage clients, families, and caregivers in clinical interventions and risk reduction strategies, working collaboratively with the interdisciplinary team.
  • Advocate on behalf of clients, families, and caregivers for appropriate services and resources.
  • Collaborate with community service providers and other stakeholders to ensure seamless care coordination.
  • Provide timely and comprehensive written assessments, treatment, and discharge reports.
  • Participate in the teaching, education, and supervision of social work students.
  • Maintain accurate and up-to-date documentation and data collection, adhering to hospital, college, and legislative standards.
  • Participate in social work meetings, peer consultation, clinical consultation, committees, treatment care conferences, and research.
  • Meet all requirements for registration with the Ontario College of Social Workers and Social Service Workers.
  • Stay current with professional standards by reviewing literature, attending educational events, and networking with colleagues.
  • Comply with hospital policies and procedures, as well as applicable legislation.
Requirements
  • Master's degree in Social Work from an accredited university.
  • Minimum of three years' experience in a mental health setting.
  • Member in good standing with the Ontario College of Social Workers and Social Service Workers.
  • Strong client- and family-centered approach.
  • Understanding of recovery-based treatment and programs for clients with severe and persistent mental illness.
  • Experience in individual, family, and group therapy/intervention, crisis intervention, psychosocial assessment, risk assessment and management, discharge planning, and liaison with community service providers is an asset.
  • Knowledge, skills, and training in concurrent disorders and Cognitive Behaviour Therapy (CBT) are preferred.
  • Knowledge of community resources, Mental Health Court, Ontario Review Board, and applicable legislated acts.
  • Demonstrate thorough knowledge of the theories and practices of a professional social worker.
  • Demonstrate ability to work both independently and in a professional interdisciplinary team environment.
  • Strong advocacy and conflict resolution skills.
  • Strong organizational skills and work ethics.
  • Valid driver's license.
  • Willingness to work variable hours, evenings, and weekends if required.
  • English language proficiency (A- level) in oral expression, oral comprehension, reading, and writing. Bilingualism (French/English) is an asset.
  • Excellent oral and written communication skills.
