Nurse Practitioner Role

Job Title: Nurse Practitioner
Job Summary:
We are seeking a highly skilled and experienced Nurse Practitioner to join our team at Trillium Health Partners. The successful candidate will be responsible for providing advanced nursing practice to patients with complex diabetes needs.
Key Responsibilities:
- Conduct comprehensive health assessments and develop outpatient care plans
- Implement care plans and evaluate patient outcomes
- Collaborate with nursing colleagues, physicians, and other healthcare professionals to provide quality care
- Develop effective partnerships with physicians and other members of the inter-professional team
- Apply a comprehensive theoretical knowledge base and approved medical directives to assess and manage patients' medical care requirements
- Provide leadership in clinical practice through mentoring and participating in formal and informal staff education
- Evaluate the care provided to patients and maintain accurate statistics for reporting
Requirements:
- Registration in the Registered Nurse Extended Class with the College of Nurses
- Experience in diabetes management is preferred
- Membership in the Registered Nurses' Association of Ontario, required
- Demonstrated excellence in practice in primary care for a minimum of 2 years
- Demonstrated experience with completing comprehensive assessments on complex clients
- Experience in evaluating quality of care and patient outcomes within the specialty population
- Demonstrated effective leadership skills
- Strong interpersonal communication skills and ability to establish positive relationships with all staff, clients, families, and community agencies
- Current Basic Cardiac Life Support (BCLS) certification
