About the Role:
We are seeking a skilled and experienced Home Care Director to join our Home Health Leadership team at Lifespace Communities. As a key member of our team, you will be responsible for delivering high-quality care to our residents and leading our team of healthcare professionals.
Key Responsibilities:
- Market home health services to residents and develop personalized care plans.
- Direct resident care, conduct assessments, and collaborate with healthcare professionals to implement and maintain care plans.
- Monitor resident conditions, medication, and nutritional needs, and update care plans as necessary.
- Ensure nursing services are provided according to resident care plans, with 24/7 access to home health services.
- Develop and maintain an on-call coverage system for nursing and resident care services.
- Participate in the development of administrative and resident care policies and procedures.
Requirements:
- Licensed Practical or Registered Nurse, as required by the state.
- Three years of experience as a nurse, with two years of administrative/supervisory experience in home health or community health.
- Completion of a bachelor's degree in nursing or related field, highly preferred.
- Completion of a master's degree program in nursing, including public health and/or administrative studies, preferred.
- Public Health Nursing certification, if required.
