Job Title: Continuing Care Assistant

Closing the Gap Healthcare Group Inc. is seeking a skilled and compassionate Continuing Care Assistant to join our team. As a Continuing Care Assistant, you will play a vital role in providing high-quality patient care and support in a home care and visiting care agency setting.

Key Responsibilities:
  • Perform various patient care tasks, including bathing, dressing, and grooming, as well as administering first aid in emergency situations.
  • Assist patients with daily living activities, such as serving meal trays, feeding patients, and making beds.
  • Provide emotional support and companionship to patients, promoting their overall well-being and quality of life.
  • Work collaboratively with healthcare professionals to develop and implement patient care plans.
  • Maintain accurate records and reports, ensuring confidentiality and adherence to regulatory requirements.
Requirements:
  • Basic Cardiac Life Support (BCLS) Certificate
  • Continuing Care Assistant Certification
  • First Aid Certificate
  • CPR Certificate
  • Valid driver's license and reliable transportation
  • Ability to work in a fast-paced environment and under pressure
  • Excellent communication and interpersonal skills
  • Ability to lift, bend, and stand for extended periods
