Job Title: Continuing Care Assistant

Closing the Gap Healthcare Group Inc. is seeking a skilled and compassionate Continuing Care Assistant to join our team. As a Continuing Care Assistant, you will play a vital role in providing high-quality patient care and support in a home care and visiting care agency setting.

Job Summary:

We are looking for a dedicated and caring individual to provide personal care and support to patients in their homes. The ideal candidate will have a strong commitment to delivering exceptional patient care and a passion for working in a fast-paced environment.

Responsibilities:
  • Provide personal care and support to patients, including bathing, dressing, and grooming
  • Assist patients with daily living activities, such as meal preparation and light housekeeping
  • Monitor patients' vital signs and report any changes to healthcare professionals
  • Administer medications and provide wound care as required
  • Transport patients to appointments and engage in activities to promote social interaction and well-being
  • Collaborate with healthcare professionals to develop and implement patient care plans
  • Maintain accurate records of patient care and progress
Requirements:
  • Basic Cardiac Life Support (BCLS) Certificate
  • Continuing Care Assistant Certification
  • First Aid Certificate
  • CPR Certificate
  • Valid driver's license and reliable transportation
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to lift and move patients as required
