Job Summary

The Clinical Coordinator, as a leader and member of the management team within Community Programs, is responsible and accountable for the coordination and direction of all patient care, bed utilization, resource management, and staffing related activities within the Transitional Care Unit (TCU) at the Southlake Regional Health Centre.

Key Responsibilities

  • Liaise with Physicians, Nurses, Staff, Students, and Volunteers to ensure seamless patient care.
  • Coordinate patient flow into and out of the TCU in collaboration with the Manager, and the interprofessional team at TCU and main site to ensure that the right patient is in the right bed at the right time.
  • Ensure resources, supplies, and equipment are available on the units at all times.
  • Lead the team in implementing evidence-based practices within the patient-centered care and interprofessional model of care philosophy.
  • In collaboration with the Manager, perform human resource responsibilities including recruitment, hiring, and ongoing performance evaluation.
  • Actively participate with the team in implementing evidence-based practices to achieve program objectives.
  • Support the development of a Patient-Centered Care environment, shared governance models, and structures, and continuous learning and education.
  • Act as the program champion in corporate initiatives, including Hand Hygiene, AIDET, IMCR, Staff/Patient Satisfaction, Hardwiring, and other corporate initiatives as assigned.
  • Actively participate in the accreditation process and assist in the development of program goals and objectives related to the accreditation process.
  • Promote inter-unit, inter-program, and inter-facility relationships to enhance team and unit performance.

Qualifications

  • Regulated Healthcare professional required.
  • Current registration, in good standing with the applicable College required.
  • Minimum five (5) years direct patient care experience.
  • Experience in a community healthcare setting is an asset.
  • Minimum two (2) years progressive administrative experience (team leader/supervisor, etc.) in a clinical setting preferred.
  • Knowledge of relevant legislation and collective agreements.
  • Strong commitment to Patient and Family-Centered Care.
  • Strong verbal and written communication skills.
  • Excellent interpersonal skills.
  • Geriatric certificate or equivalent.
  • Minimum of five (5) years recent and relevant Geriatric or Chronic Disease Management experience an asset.
  • Experience in adult education, mentoring, leadership, and coaching.
  • Demonstrated excellence and experience in the area of Geriatric and Chronic Disease care.
  • Demonstrated excellence in interpersonal, communication, team building, facilitation, and coaching skills that encourage positive interactions among all members of the health care team.
  • Demonstrated understanding of systems and system analysis techniques, value analysis, continuous improvement principles, and tools.
  • A strong customer and patient/caregiver focus philosophy in all interactions.
  • Ability to introduce and effectively facilitate change.
  • Demonstrated critical thinking, decision-making, and organizational skills.
  • Demonstrated commitment to professional development.
  • Demonstrated ability to monitor attendance and work with staff that has attendance issues to set goals to effect positive change in their attendance at work.
  • Demonstrates proven ability to manage conflict resolution.
  • Experience in leading and/or participating in research-based studies.
  • Experience with co-ordinating care in both inpatient and outpatient clinics.
  • Proven ability to attend work regularly as per the Attendance Support Policy #HA30 and Procedure #HA35.
