Care Coordinator for On-Site Support Services

We are seeking a Care Coordinator for On-Site Support Services to join our team at Chanakya Upmanu. The successful candidate will be responsible for supporting Veterans in transitioning their military skills and experience into the civilian job market.
Job Summary:The Care Coordinator will work with our clients to provide on-site support services, ensuring that they have the necessary resources and tools to succeed in their roles.

Key Responsibilities:- Providing on-site support services to clients
- Assisting clients with resume building and interview preparation
- Connecting clients with local resources and services
- Monitoring client progress and providing feedback
To be successful in this role, you will need:
- A high school diploma or equivalent
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
Experience working with vulnerable populations and knowledge of local resources and services would be an asset.
What We Offer:We offer a competitive salary range of $45,000 - $55,000 per year, depending on experience.
