Job Title: Rehabilitation Service Worker

We are seeking a highly skilled and dedicated Rehabilitation Service Worker to join our team at Chezranti Community Centre Ltd.

Job Summary:

The Rehabilitation Service Worker will be responsible for providing rehabilitation services to clients, including developing and implementing rehabilitation programs, conducting assessments, and providing support and guidance to clients.

Key Responsibilities:
  • Develop and implement rehabilitation programs for clients, including individualized plans and group programs.
  • Conduct assessments and evaluations of clients to determine their needs and goals.
  • Provide support and guidance to clients, including counseling and education.
  • Collaborate with other healthcare professionals and community agencies to ensure comprehensive care for clients.
  • Maintain accurate records and reports of client progress and program outcomes.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
  • Education: College/CEGEP degree in a related field.
  • Experience: 3 years to less than 5 years of experience in a rehabilitation setting.
  • Skills: Strong communication and interpersonal skills, ability to work independently and as part of a team, and ability to maintain confidentiality.
