We are seeking a skilled and compassionate Nurse Care Specialist to join our team at ParaMed Home Health Care. As a key member of our community nursing team, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities:
- Participate in scheduled visits in the community to assess and address patient needs within your scope of practice.
- Provide safe and effective nursing care, using appropriate assessment tools and techniques to identify individual patient needs.
- Develop and implement personalized care plans, working collaboratively with patients and their families to achieve optimal health outcomes.
- Engage patients in their care, building rapport and trust through a patient-centered approach.
- Maintain accurate and up-to-date patient records, documenting all interactions and care provided.
Requirements:
- Registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse (RPN) or Registered Nurse (RN) in good standing.
- Valid driver's license and access to a reliable vehicle.
- Experience in delivering community care is an asset.
- Reliability, flexibility, and a strong commitment to providing exceptional patient care.
