About the Role:

We are seeking a skilled Caregiving Specialist to join our team. As a Caregiving Specialist, you will be responsible for providing high-quality care to patients in their homes.

Responsibilities:

  • Visit patients in their homes for one-on-one visits to assess their physical, psychosocial, and environmental needs.
  • Complete initial and ongoing assessments to identify the needs of home health patients.
  • Administer medication as prescribed by the patient's physician.
  • Educate patients and their families on proper home health care strategies and procedures.
  • Provide effective communication to patients/clients, their family members, team members, and other health care professionals.

Requirements:

  • LPN/RN license.
  • First Aid Certificate and Cardiopulmonary Resuscitation (CPR) Certificate.
  • Police Criminal Record Check with Vulnerable Sector Screening.
  • Community experience is an asset.

Additional Requirements:

  • Must have access to a reliable vehicle and be willing to travel in the community.
