Clinical Pharmacist, Renal Position

We are seeking a highly skilled Clinical Pharmacist to join our Renal Program at Fraser Health Authority. The successful candidate will provide clinical pharmacy services, including individualized patient medication therapy, patient counseling, and consultation with healthcare providers to develop and discuss therapeutic plans.
Key Responsibilities- Consult with physicians, nurses, and other healthcare providers to develop and discuss therapeutic plans.
- Individualize patient medication therapy by referencing standard literature, reviewing patient charts and laboratory test results, and meeting with patients and healthcare teams to assess clinical responses.
- Provide patient medication counseling regarding drug therapy, reviewing reference literature, summarizing information, and meeting with patients and their families to provide and explain information and plans.
- Assist the healthcare team in developing drug therapy guidelines for individual disease conditions by reviewing current literature, compiling and analyzing information, providing suggestions and recommendations, and monitoring the effectiveness of current guidelines on patient healthcare outcomes.
- Participate in ward rounds, program team meetings, and provision of pharmacy services at the unit level to enhance patient care.
- Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
- Completion of an accredited Pharmacy Residency Program or an equivalent combination of education, training, and experience.
- Broad knowledge of organization policies, procedures, and standards of care.
- Knowledge of evidence-based clinical pharmacy practice, medication use evaluation, research processes and methodology, and drug distribution processes in a hospital setting.
- Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision, communicate effectively both verbally and in writing, establish and maintain effective working relationships with other healthcare practitioners, patients, family members, and the public, organize and prioritize work, operate related equipment, and perform physical tasks.
