Job Summary:

We are seeking a skilled and compassionate Community Health Nurse to join our team in the Owen Sound area. As a Community Health Nurse, you will have the opportunity to work in a variety of settings, including in-home, clinics, and retirement homes, providing exceptional nursing services to patients of all ages.

Key Responsibilities:

  • Develop and maintain effective care plans for patients, working independently or in collaboration with the Home and Community Care Support Services (HCCSS)
  • Provide high-quality nursing services to patients in their homes, clinics, and other settings
  • Communicate and liaise with patients, family members, and other healthcare professionals to ensure seamless care coordination
  • Perform other clinical duties as required

Requirements:

  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills
  • A valid driver's license and vehicle (regular travel required)
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record, including 2-step TB test
  • Vulnerable sector check and clear background check (required for patient safety)
