Job Summary
Vancouver Coastal Health is seeking a Registered Nurse (RN) or Registered Psychiatric Nurse (RPN) to join the Community Transition Care Team in Vancouver, BC. As a member of our team, you will provide holistic nursing care to clients with a variety of acute and chronic illnesses/diseases, including mental health and substance use, communicable diseases, and/or general health issues.
Key Responsibilities
- Provide physical and mental health assessments, establish, coordinate, review, and update care plans, observe, monitor, and evaluate client progress and response to treatments, make referrals and support transition between services as needed.
- Use the principles of harm reduction, provide education and counselling, and administer prescribed treatment, medication, and intravenous therapy in a safe and effective manner as required.
- Collaborate with site partners in a culturally safe manner, including community-based organizations, community members, and people actively using substances.
Qualifications
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One (1) year recent related clinical nursing experience providing care and/or treatment services to the designated client population through a harm reduction, holistic mental health and substance use lens, or an equivalent combination of education and experience.
