About the Role

Vancouver Coastal Health is seeking a skilled Community Health Nurse to join our Home Health team at Pender Community Health Centre in Vancouver.

Key Responsibilities

  • Provide direct care to clients with chronic disease, frailty, and other conditions requiring support to live independently at home.
  • Use standardized assessment and screening tools to assess, plan, implement, and evaluate care, including home support services and accessing community resources.
  • Establish and maintain therapeutic relationships based on mutual respect and client-centered care.
  • Support clients and families to achieve and maintain optimal health based on client needs and goals.

About Home Health Programs

Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative, or rehabilitative health care needs.

Qualifications

  • Graduation from a recognized program for Licensed Practical Nurses.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives.
  • Two years recent experience working as a Licensed Practical Nurse providing care in acute care and one year in community settings or an equivalent combination of training, education, and experience.
