Geriatric Care Coordinator

The Community Health Nurse plays a critical role in delivering high-quality care to clients who require complex geriatric and adult care. This role involves working with clients and their families to assess their needs, develop individualized care plans, and coordinate care services and referrals.
The Community Health Nurse works collaboratively with other healthcare professionals to ensure that clients receive seamless care. They conduct initial and ongoing assessments, develop and implement care plans, and facilitate care conferencing to review client care plans and determine next steps.
In addition to these key responsibilities, the Community Health Nurse provides education and support to clients and their families on health-related topics and self-care practices. They also maintain accurate and up-to-date clinical records and statistical information.
Key Requirements
To be successful in this role, you will have:
- A current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
- One year recent related clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions in an acute or community/outpatient care setting
- Valid BC Driver's license and access to a personal vehicle for business-related purposes
