Job Overview

The Group Health Centre (GHC) is seeking a skilled Nurse Practitioner to join our team in the Access Care Clinic. As a key member of our multidisciplinary team, you will provide high-quality patient care, working collaboratively with other healthcare professionals to deliver exceptional results.

Responsibilities

  • Deliver primary care services to patients, including urgent care, episodic care, and chronic disease management.
  • Work closely with other healthcare professionals to ensure seamless patient care and referrals.
  • Provide education and support to patients and their families, promoting healthy lifestyles and disease prevention.
  • Participate in quality improvement initiatives, contributing to the development of best practices and policies.
  • Collaborate with other healthcare teams to ensure continuity of care and optimal patient outcomes.

Requirements

  • Valid Extended Class Certification of Registration with The College of Nurses of Ontario (CNO)
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong critical thinking and problem-solving skills
  • Commitment to providing high-quality patient care and promoting a positive work environment
