We are seeking a skilled Rehabilitation Specialist to join our team at St. Paul Abilities Network (Society). As a key member of our community rehabilitation team, you will play a vital role in supporting individuals with disabilities to achieve their goals and live independently.

Key Responsibilities
  • Provide education and support to clients to help them develop the skills and knowledge needed to live independently.
  • Assess clients' needs and develop personalized plans to address their goals and objectives.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support.
  • Conduct regular assessments and evaluations to monitor clients' progress and adjust plans as needed.
Requirements
  • Certificate or diploma from a program of 3 months to less than 1 year.
  • 7 months to less than 1 year of experience in a related field.
  • Criminal record check, Child Abuse Registry check, Driver's validity licence check, and Adult Abuse Registry check.
