About the Role

We are seeking a highly skilled and experienced Community Health and Harm Reduction Supervisor to join our team at Homes First Society. As a key member of our organization, you will play a vital role in coordinating, planning, and evaluating harm reduction initiatives that promote comprehensive support, referrals, and crisis interventions for our clients.

Key Responsibilities
  • Harm Reduction Needs Assessments: Conduct thorough needs assessments to identify the harm reduction needs of our clients and develop effective strategies to address them.
  • Staff Training and Development: Educate and train staff on harm reduction guidelines, procedures, and standards to ensure consistent service delivery and effective use of resources.
  • Team Supervision: Supervise a team of staff members and provide guidance and support to ensure they are equipped to provide high-quality services to our clients.
  • Client Support and Referrals: Develop and implement complex case plans to provide comprehensive support, referrals, and crisis interventions for clients with mental health, addictions, and physical health needs.
  • Partnership Development: Develop and maintain partnerships with community organizations, healthcare providers, and other stakeholders to ensure effective harm reduction initiatives and comprehensive support services for our clients.
  • Program Development and Implementation: Develop and implement new harm reduction and health programming to address the evolving needs of our clients.
  • Client Engagement and Participation: Engage with clients to understand their needs and preferences and involve them in the planning, implementation, and evaluation of harm reduction initiatives.
  • Crisis Intervention and Support: Provide crisis intervention and support services to clients in need, including interventions involving the Mental Health Act, MCIT, and Form 2s.
  • Collaboration and Liaison: Collaborate with hospitals, healthcare providers, and other stakeholders to ensure seamless care coordination and effective discharge planning for our clients.
  • Reporting and Documentation: Develop and maintain accurate and comprehensive reports and documentation to track client progress, program outcomes, and service delivery.
Requirements
  • Education and Experience: A related diploma or certificate, or equivalent combination of education, training, and experience, with at least one year of supervisory experience in a unionized environment.
  • Experience with Harm Reduction: At least three years of significant experience working with people who are/have experienced homelessness, use drugs, racialized populations, Indigenous Peoples, people who engage in sex work, have mental health issues, and have had involvement with the court/jail/prison systems.
  • Communication and Interpersonal Skills: Excellent communication and interpersonal skills, with the ability to work effectively in a collaborative team environment.
  • Time Management and Organization: Excellent time management and organization skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
  • Certifications and Training: Certificates in First Aid, CPR, Crisis Prevention and Intervention, and other relevant certifications and training.
