Key Responsibilities- Develop and maintain effective care plans for patients, working independently or in collaboration with the Ontario Health atHome team.
- Provide skilled nursing services to patients, including in-person and virtual visits, to promote optimal health and well-being.
- Communicate effectively with patients, family members, and other healthcare professionals to ensure seamless care coordination.
- Perform other clinical duties as required, including administering medications, wound care, and vital sign monitoring.
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
- Current College of Nurses of Ontario (CNO) registration in good standing.
- Strong commitment to delivering high-quality patient care and making a positive impact in the community.
- Excellent interpersonal and communication skills.
- A valid driver's license and reliable vehicle for regular travel.
- CPR certification and current immunization record, including a 2-step TB test.
- Vulnerable sector check and clear background check.
