Job Summary
Halton Healthcare is seeking a skilled Registered Practical Nurse to join our team in providing high-quality care to complex transitional care populations. As a key member of our healthcare team, you will be responsible for delivering patient-centered care, advocating for the elderly, and working collaboratively with other healthcare professionals to ensure the best possible outcomes for our patients.
Key Responsibilities
- Provide direct patient care, including medication administration, physical assessments, and wound care.
- Advocate for the elderly, ensuring their needs are met and their rights are respected.
- Collaborate with other healthcare professionals to develop and implement individualized care plans.
- Maintain accurate and detailed records of patient care, including electronic documentation.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements
- Current registration with the College of Nurses of Ontario.
- Current Heart & Stroke Approved BLS Provider Card upon hire.
- Experience working with and advocating for the elderly.
- Medication administration certification and experience.
- Physical assessment and wound care skills.
- Enteral feeding experience and maintenance of IV lines.
- Experience with electronic documentation.
- Satisfactory employment and attendance record.
