Job Title: Case Manager

We are seeking a skilled and compassionate Case Manager to join our team at the Canadian Mental Health Association, Muskoka-Parry Sound Branch. The successful candidate will be responsible for providing support and guidance to individuals and families in our community.

Key Responsibilities:
  • Provide case management services to clients, including assessment, planning, and coordination of care.
  • Develop and implement individualized plans to meet the unique needs of each client.
  • Collaborate with community agencies and healthcare providers to ensure comprehensive support.
  • Provide education and support to clients and their families on mental health issues and coping strategies.
  • Document client progress and maintain accurate records.
Requirements:
  • Bachelor's degree in a related field, such as social work, psychology, or a related field.
  • 1-2 years of experience in a related field, preferably in a community or social services setting.
  • Aboriginal studies or equivalent experience an asset.
  • Valid driver's license and reliable transportation.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
