Nurse Practitioner

The Registered Nurse is a key member of the VON Canada team, responsible for providing high-quality, client-centered care to individuals and families in their homes and community. This role requires a strong commitment to delivering evidence-based care, promoting health and well-being, and supporting independence.
Key Responsibilities- Manage and coordinate care, evaluating health outcomes and educating clients and families to meet health goals.
- Provide all levels of nursing care, from stable clients with predictable outcomes to those with acute, complex, and rapidly changing health needs.
- Develop and implement care plans, incorporating discharge planning as needed, and ensuring services meet client needs.
- Collaborate with clients, families, and caregivers to promote involvement in determining health and service needs.
- Support the health and well-being of clients and families, promoting independence and self-care through teaching and health promotion.
- Work collaboratively with other healthcare professionals and the community to promote nursing best practices and influence policy review and development.
- Licensed as a Registered Nurse in the province of practice.
- Canadian Nurses Association certification in Community Health Nursing is an asset.
- Clinical practice experience and experience in community health are assets.
- Strong written and verbal communication skills, with the ability to engage effectively with clients, staff, and the public.
- Ability to work independently, problem-solve, and adapt nursing interventions as needed.
