About the Position:

Key Responsibilities:

  • Provide professional assessment and coordination of patient care in the community and homes
  • Develop and maintain effective care plans with respect to Community Health Services
  • Collaborate with Community Support Workers in the field and provide coaching and supervision
  • Contribute to promoting health and safety in the workplace and community

Requirements:

  • Master's Degree in Occupational Therapy from a recognized university
  • Proven computer skills, including MS Word and Outlook
  • Excellent English verbal and written communication skills
  • French language skills are an asset
