Job Summary

WoodGreen is seeking a skilled Clinical Counsellor to join our Caregiver Support and Wellness team. As a Clinical Counsellor, you will provide psychosocial assessments, case management, and counselling services to caregivers supporting individuals with geriatric, mental health, and cognitive issues. You will work closely with primary care and service providers to ensure positive health-related outcomes and collaborate with other teams to provide integrated care.

Key Responsibilities
  • Provide psychosocial assessments and case management services to caregivers
  • Develop and implement service plans with caregivers and their care recipients
  • Provide counselling and support services to caregivers in homes and community settings
  • Advocate on behalf of clients and caregivers
  • Link clients and caregivers to community resources and facilitate referrals
  • Facilitate therapeutic groups and provide mentorship to social workers and volunteers
  • Develop relationships with community agencies and seniors' groups
  • Participate in team meetings and ongoing training and education

Requirements
  • Masters of Social Work, Psychology, Counselling, or postgraduate degree in a relevant field
  • Registration in good standing with the Ontario College of Social Workers and Social Service Workers or equivalent regulatory college
  • 3+ years of related experience in geriatrics and/or social work
  • Experience working with older adults and seniors with dementia, mental health, and substance use issues
