Licensed Practical Nurse

The Licensed Practical Nurse (LPN) serves as a key member of our care team, reporting to the Director of Care and/or designate. As a role model and team leader, this position is responsible for coordinating care delivery for our residents, ensuring individualized and resident-centered care.
Key Responsibilities:- Assessment and Care Planning: Conduct thorough assessments of residents' needs, implementing and evaluating individualized care plans to maximize their well-being.
- Resident Rights and Holistic Care: Ensure residents' rights are respected and upheld, adhering to a holistic philosophy of wellness that addresses their physical, emotional, and social needs.
- Interdisciplinary Collaboration: Liaise with physicians, community agency representatives, families, visitors, volunteers, and other disciplines to ensure seamless care delivery.
- Wound Care and Skin Integrity: Assess and treat wounds on the unit, providing guidance and support to care staff on strategies to prevent skin breakdown.
- Medication Administration: Administer prescribed medications and treatments within the scope of practice, as defined by the BC College of Nurses and Midwives (BCCNM).
- Monitoring and Reporting: Continuously monitor and report changes in residents' physical and psychological conditions, documenting changes in their health records and notifying families and physicians as needed.
- Interdisciplinary Care Conferences: Participate actively in care conferences to ensure current and individualized care plans are in place.
- Administrative Support: Provide administrative support, including admissions, discharges, and maintaining confidentiality, in accordance with established standards.
- Staff Development and Education: Participate in the orientation of new staff, community programs, meetings, committees, and educational in-services, promoting a culture of continuous learning and growth.
- Coaching and Direction: Provide coaching and direction to care staff as needed, using in-the-moment education to improve resident care and completing Point of Care (POC) documentation.
- Knowledge and Competency: Maintain up-to-date knowledge of community policies and procedures, particularly those concerning emergency procedures, equipment management, and safe work practices.
- Education and Licensure: Graduate of an approved program for Practical Nursing and holds a current license with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
- Certifications and Competencies: Valid Occupational First Aid Level 1 certification and successful annual Resident Assessment Instrument (RAI) competency testing.
- Experience and Abilities: Previous experience in a long-term care facility is an asset, and the ability to read, write, and speak English proficiently is required.
- Computer Skills: Proficiency in basic computer programs, including Word, Excel, and Outlook.
- Collaboration and Teamwork: Demonstrates collaborative and team-working skills, with the ability to work effectively in a fast-paced environment.
- Physical Demands: Must be in good health, physically able to perform repetitive tasks, lift, squat, bend, stand, and walk as required to perform the duties of the position.
