Job Summary

Halton Healthcare is seeking a highly skilled and compassionate Program Coordinator to join our team. As a key member of our Supportive Housing Program, you will be responsible for coordinating services for seniors at our supportive housing site.

Key Responsibilities
  • Assess client eligibility and needs, developing personalized care plans to ensure optimal support.
  • Coordinate service delivery, supervising contracted staff to ensure seamless execution.
  • Lead the on-site team to achieve operational and client care objectives, fostering a collaborative and supportive work environment.
  • Develop and maintain relationships with community partners, ensuring access to necessary resources and services.
  • Provide crisis intervention and support, ensuring the well-being and safety of clients.
Requirements
  • College-level diploma in the human services field or related discipline.
  • Education and/or experience in gerontology, with a strong understanding of systems and services for seniors.
  • Excellent oral and written communication skills, with the ability to build strong relationships with clients, staff, and community partners.
  • Strong organizational and analytical skills, with the ability to prioritize tasks and manage multiple projects.
  • Computer proficiency, with experience in relevant software and systems.
