Job Summary:
We are seeking a skilled and compassionate Nurse Care Specialist to join our team at Southbridge Care Homes. As a key member of our healthcare team, you will play a vital role in providing high-quality care to our residents.
Key Responsibilities:
- Deliver resident-centered care that meets the physical, emotional, and social needs of our residents.
- Assess and monitor resident health status, develop, implement, and evaluate individualized care plans.
- Supervise and direct nursing and personal care staff, ensuring that care is delivered in accordance with established standards and policies.
- Collaborate with interdisciplinary teams to coordinate care and services, ensuring seamless transitions and optimal outcomes.
- Provide education and support to residents, families, and staff on health-related topics and care practices.
Requirements:
- Current certificate of competence from the College of Nurses of Ontario.
- Registered/eligible for registration with the Registered Nurses Association of Ontario.
- Previous experience in long-term care, with a strong understanding of care practices and policies.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Bilingualism in English and French is an asset.
